National Championship Student Drug Testing Consent form
Include this form as part of your student-athlete onboarding practice. Your institution should have a completed consent form on file for every student-athlete.

Medical Exemption Form
This form is required when a student-athlete has a medical condition that requires the use of a banned substance. For those using anabolic agents and/or peptide hormones, the form must be submitted to the NAIA national office prior to any competition. For all other banned substances, this form is required should an institution appeal the results of a student-athlete testing positive for a banned substance.

COMPETING IN A CHAMPIONSHIP
A completed NAIA drug testing consent form is required for every student-athlete on a post-season roster. The athlete will not be able to participate in post-season play unless his or her drug testing consent form has been completed and is on file with the institution.

For sports that typically submit their post-season roster online, an acknowledgement statement has been added regarding the consent forms. By checking a box, the submitter is certifying that all student-athletes on the roster have a signed consent form on file with the institution.

It has been standard practice for cross country and track and field teams to submit post-season rosters manually. This process will now require the NAIA Certification of Consent Form be included with the manual submission documentation, certifying that all student-athletes on the roster have a signed consent form on file with the institution.
