Submit a Required Forfeit

The submission of a required forfeit are for cases when a forfeit has been required by a national committee (National Eligibility Committee, Competitive Experience Committee, or National Coordinating Committee). In such cases, it was determined that an institutional violation occurred in regards to the use of an ineligible athlete or other rules violations that require forfeits.

• The athletics director or faculty athletics representative of the institution must submit an NAIA Required Forfeit Form.
• A required forfeit must be submitted within 10 days of the notification that comes from a national committee.
• A copy of the forfeit notification letter(s) that are distributed to opposing institutions must accompany this form.
• Electronic submission of the official form is required.
• A link explaining the forfeit procedure for a particular sport can be found by each sport listed below.

Request a Forfeit

The submission of a requested forfeit falls under the purview of NAC Policy XII, Article D.

• The athletics director or faculty athletics representative of the offended institution must submit an NAIA Forfeit Request Form (LINK).
• The request must be submitted within 10 days of the forfeit in question or prior to the start of postseason competition, whichever is earlier.
• A copy of the contract or bracket assignments for postseason play must accompany the written request for the forfeit.
• Electronic submission of the official form is required.
• A link explaining the forfeit procedure for a particular sport can be found by each sport listed below.
